- Expert Reports – Care and Case Management
- Training for support workers and carers of individuals with acquired brain injury
What does a case manager do?
All our case managers aim to do one thing – Optimise Potential. This process can include:
- Providing an individual assessment to identify each client’s individual needs
- Identifying appropriate goals
- Establishing a comprehensive care a therapy package tailored to meet individual need
- Provision of specialist equipment
- Ongoing risk assessment
- Ongoing advocacy for the client and support to family and friends
- Liaising with other agencies to secure funding
